  • Notify Relevant Parties: Inform appropriate personnel, such as supervisors, managers, your health and safety representative, RIDDOR (if required) and any other relevant stakeholders, about the accident. Ensure that they are aware of the situation and can provide necessary support.

  • Collect Information: Gather as much information as possible about the accident. This can include interviewing witnesses, taking photographs or videos of the scene, examining equipment or machinery involved, and reviewing relevant documents, such as work procedures, maintenance records, or training logs.

  • Analyse the Sequence of Events: Leading up to the accident. Identify any contributing factors, such as unsafe acts, unsafe conditions, equipment failures, human error, lack of training, or communication breakdowns. Consider both immediate and underlying causes.

  • Identify Root Causes: Dig deeper to identify the root causes that underlie the accident. Root causes are the underlying factors that, if addressed, can help prevent similar incidents from occurring in the future. 

  • Report Findings: Prepare a detailed accident investigation report that summarises the findings. Include a description of the incident, the sequence of events, contributing factors, root causes, and any recommended corrective actions. We ensure that the report is accurate, objective, and clear.

  • Implement Corrective Actions: Based on the investigation findings. These actions should address the root causes and aim to prevent similar incidents from occurring in the future. We assign responsibility for each corrective action and establish a timeline for completion.

  • Health and Safety Training and Communication: We share the lessons learned from the health and safety accident investigations with all relevant personnel and communicate the findings to raise awareness and promote a culture of safety. We also provide health and safety training to prevent similar incidents in the future.

  • Review and Improve: Periodically we will review the effectiveness of implemented corrective actions and evaluate their impact on health and safety performance. We use the findings to continuously improve health and safety policies and procedures, health and safety training, and risk management strategies within your organisation.
